Who is eligible for Circle Around Families?

Children who are eligible for services include all of the following criteria:

• Are 6-18 years of age;
• Are at-risk of being removed from home;
• Live in Hammond, East Chicago, or Gary, Indiana;
• Have been diagnosed with serious emotional disorder (SED);
• Are involved in or need two or more human services systems.

Referral agreement and release of information

You will be asked to sign a referral agreement, which confirms that you understand the referral process and agree to participate in that process.

In order to help you discover the services that will be most helpful, we need to talk with the people who know your child best. By signing a release of information, you give us permission to share information. You will be asked to specify who you want to include on the release form.

Making referrals

Parents, schools, mental health agencies, parent advocates, and the juvenile court make the primary referrals to Circle Around Families. You may request a referral form by calling (219) 757-1822. The completed form can be returned by fax at (219) 738-5283 or mailed to:
